Melissa Mahoney, LSW
Medical Social Worker
I am a licensed social worker who graduated from Rutgers University with a Master’s in Social Work and a Certificate in Gerontology. I have worked with the geriatric and disabled population since 2012. I have served as Director of Social Services in two sub acute rehab facilities, where my main role was to discharge patients safely back into their homes. It wasn’t until I took a position in homecare that I realized the extent of the needs of the aging population. During my years working in homecare, I saw the lack of support, advocacy, and knowledge of community resources to address their needs. Many of my clients were vulnerable, and their safety and wellbeing were compromised, leaving their loved ones frustrated and worried. Yet, they were adamant they did not want to be “put into a nursing home.” Since then, I worked tirelessly to not only allow seniors to safely age in place, but to also thrive in the community and enjoy their later years. This is truly my passion.
I recognize the importance of taking the time to build a rapport with clients and their loved ones in order to fully assess their needs and formulate an effective care plan. What sets me apart is my ability to build relationships with individuals of varied socioeconomic backgrounds, and use tools such as patience, flexibility and creativity to obtain solutions to their needs. Most importantly, I promote dignity and respect, and I make it a point to include the aging individual to participate in their own care plan.
